Creating a polished, user-friendly monday.com form is easier than you might think. With just a few adjustments, you can customize the layout, apply your branding, and design an easy-to-use form that will encourage completion.
Here’s how to customize and design your form in SuperForm.
Use Form Pagination
Long forms can overwhelm users, but pagination breaks them into smaller, more digestible sections. For example, dedicate one page to the user’s personal details and another to order information.
- To add a page: go to the Fields section and click Add Page

- Reorder questions by dragging and dropping, or move several questions at once with multi-select
- For additional clarity, add titles to each page. For a cleaner look, hide the page titles in the Header Visibility section.

- Pro tip: SuperForm page titles support HTML styling, allowing you to highlight important cues and guide users through the form.
Use Columns to Simplify Long Forms
SuperForm makes it easy to place two questions side by side, which shortens the appearance of longer forms and creates a comfortable user experience. For example:
- Display First Name next to Last Name.
- Place Zip Code next to Address.
To set this up:
- Go to the page preview.
- Click the plus icon beside a field.
- Choose the second field you want to display alongside it.

Adjust Layout and Alignment
Control exactly how your form is displayed:
- In the Layout tab, choose to center, left-align, or right-align the form.
- Set the language direction to left-to-right or right-to-left depending on the language of the form.

- Note: Language direction adjustments appear in the live form but not in the preview builder.
Customize Your Form Design
The Design tab gives you full creative control:
- Choose from a solid background color, a pre-designed template, or uploading a custom image.
- By default, your company logo will display at the top of the form. Change the logo by uploading a new file.
- To hide the logo completely, toggle it off
- Style the submit button, your most important call-to-action: change its color to match your brand and edit the button text to provide a strong call-to-action.
- Pro tip: Blue buttons tend to get more clicks than any other color.

Add a Form Title
At the top of the form builder, you can edit the form’s title and subtitle. Copy and paste HTML into the fields to style text, apply colors, bolding, or even include links.
- Pro tip: To add HTML text to form titles or subtitles, create the HTML in any online HTML generator. Format the font, size, color, and more. Then, copy and paste the HTML code into the SuperForm editor to generate designed text directly in your monday form.
Add Images, Dividers, Text Blocks & More
SuperForm widgets let you customize and structure your forms beyond basic data fields, adding elements like text blocks, images, dividers, videos, and iFrames to create a clearer, more interactive experience. SuperForm widgets help you guide users through the form, provide context or instructions right where they’re needed, and make your forms as easy to follow as possible.
- Hover over the field under which you would like to add the widget. Click the plus (+) button.
- Select the widget you’d like to add

Create a Personalized Thank You Screen
With SuperForm, you won’t just customize the form experience, you can also customize the follow-up experience.
- In the Settings tab, scroll to Thank You Screen.
- Edit the success message, description, and button text as desired.
- Choose whether to show the success animation.
- You can also choose to redirect users to a custom URL instead of showing a message.

Putting it All Together
By the end of the process, here’s how you can customize your SuperForm from A to Z:
- Your logo and brand colors.
- Titles and subtitles enhanced with HTML styling.
- Clean organization with pagination and side-by-side columns.
- A polished, customizable thank-you page.
