Customer onboarding should be a smooth experience for both customers and team members. When customer information, approvals, medical forms, waivers, and other important updates are handled separately, details are likely to get missed and customers are likely to get stuck before completing the onboarding process. A professionally built customer onboarding template combined with a powerful SuperForm can solve problems of onboarding abandonment, broken data flows, and all-around frustration.
The Problem
Most customer onboarding workflows look simple. It sounds obvious: A new customer signs up, fills in their details, submits the required information, and moves to the next stage.
In real life, however, the process is often more complicated than it seems.
Teams need to confirm the customer’s email address, collect missing information, review required forms, check whether any responses require attention, save signed documents, notify the right people, and update the customer’s lead status, among other confirmations. When these steps are handled manually, the team spends too much time chasing customers, copying data between systems, checking files, and reminding internal stakeholders to act. And, of course, data gets lost in the manual process.
That creates several common problems:
- Customer records become incomplete or inconsistent
- Internal teams lose track of who has submitted what
- Sensitive or required information is harder to manage
- Customers need repeated follow-ups
- Important red flags can be missed
- Signed documents may not stay connected to the right customer record
- The onboarding process depends too heavily on manual handoffs
For teams that onboard customers regularly, a broken process is not just a time drain. It affects speed, accuracy, compliance, and the entire customer experience.
The Solution
A pre-configured onboarding template combined with SuperForm, SuperMail, and monday.com automations baked right in gives teams an organized, ready-to-use workflow inside monday.com. The workflow is built around one central customer board. Each customer has a single item that moves through the onboarding process based on status.
Instead of building the process from scratch, teams can start from a complete onboarding structure and customize it to fit their own customer journey. In the example video which features onboarding for a running club, the board includes groups for email approval, data update, leads, and active customers. Medical clearance and liability waivers are mandatory before a participant can become an active customer.
The same structure can be adapted for many onboarding processes, including customer intake, client onboarding, vendor onboarding, partner registration, membership enrollment, training signups, compliance workflows, and more.

How the Workflow Works
1. A customer is added to the onboarding board
The process starts when a new customer is created on the monday.com board. From there, automations keep the onboarding process moving ahead smoothly.

Instead of asking the team to manually create a form link or send a request to update information, the template generates a customer-specific SuperForm link automatically, which is automatically added to the monday.com board. This custom link is critical because it enables every customer to update his or her information directly in the board, without requiring manual updates and without accidentally providing access to anyone else’s information.

2. The customer verifies their email
Email verification is a central part of the onboarding process, and this template makes email verification almost effortless.
Using SuperMail, the customer receives a verification email with a status button that can be used to confirm their email. When they click the button, the system confirms the email address and and automation moves the customer to the next stage of the onboarding process, triggering the SuperForm onboarding process.
This is useful for teams that need to confirm that customer communication details are accurate before sending forms, documents, next steps, or payment instructions.
3. The customer receives a personalized update form
Once the customer reaches the data update stage, an automated email is sent with a link to their personalized SuperForm.
The form is configured in update item mode, which means the customer is updating their existing record rather than creating a new one. This keeps the board clean and gives the team one organized source of truth for each customer.

In the example, the customer can see their name and email address, but those fields are set to view-only. This protects data integrity by letting the customer confirm the information without changing key identifying details.
The customer can then update fields such as:
- Phone number
- Date of birth
- Address
- Required onboarding details
- Medical questionnaire responses
- Digital signature
This gives the customer a comfortable, time-saving experience while ensuring the data flows back into the correct monday.com item.
4. Required fields prevent incomplete submissions
An error-free onboarding process requires the submission of all necessary information. If customers can skip required questions, the process breaks, and the onboarding team is left struggling to fill in the blanks.
In this workflow, required fields are configured directly in SuperForm. In the example, the medical questionnaire is placed on its own page, and every field in that section is marked as required.
This helps teams collect the information they need before the customer can move forward.
The form can also be divided into clear pages, such as:
- Personal details
- Medical or compliance questions
- Terms and conditions
- Liability waiver
- Signature
- Final review
This makes the experience easier for the customer and helps reduce incomplete or inaccurate submissions.

5. Digital signature
In the running club example, the final step is a liability waiver. The customer reviews the waiver and provides a digital signature before submitting the form.
This is valuable for any workflow that requires acknowledgement, consent, approval, or manual sign-off.
Examples include:
- Liability waivers
- Vendor agreements
- Client onboarding confirmations
- Policy acknowledgements
- Partner terms
- Training participation agreements
- Medical declarations
- Compliance forms
Because the signature is collected as part of the same form, the signed document stays tied to the customer’s onboarding record and is easily accessible at all times in case of audit or other needs.
6. PDF summary
After the form is submitted, the customer can download a PDF summary containing their submitted details, questionnaire responses, and signature.
The completed liability PDF is also attached directly to the customer’s item in monday.com.
For more advanced document needs, the workflow can also use SuperForm’s custom PDF function. Teams can upload a PDF template and drag monday.com columns into the right places to create a polished, branded PDF based on form answers and board data.
7. Trigger alerts on questionable submissions
Some onboarding information requires immediate attention.
In the running club example, medical questionnaire responses are analyzed automatically. If critical data is detected, the system triggers a red flag and sends an automated alert to the relevant team members.
This makes the workflow useful for processes where certain responses need review before the customer can move forward.
Examples include:
- Medical concerns
- Compliance risks
- Missing documents
- High-priority customer requests
- Vendor risk indicators
- Incomplete legal information
- Special handling requirements
Instead of asking the team to manually inspect every form, monday.com can automatically call attention to the items that need attention.
Apps and Features Used
SuperForm
SuperForm powers the customer-facing form experience. In this use case, it is used to update existing monday.com items, create personalized customer links, display view-only fields, collect required data, split the form into pages, capture digital signatures, and generate PDF summaries.
Key SuperForm features used:
- Update existing item mode
- Unique customer links
- View-only fields
- Required fields
- Multi-page forms
- Text boxes for waiver content
- Digital signature collection
- Export to PDF
- Custom PDF generation
- File column attachment
SuperMail
SuperMail supports the email verification step and can be used to send customers the right emails at the right time.
In this workflow, SuperMail sends a verification email with a status button. When the customer clicks the button, the system confirms the email address and updates the status in monday.com, triggering the next stage of the onboarding process.
Key SuperMail features used:
- Automated email sending
- Status button capability
- Email verification flow
- Customer communication from monday.com
- Status updates based on customer action
monday.com Automations
monday.com automations turn a standard monday.com board into a powerhouse customer onboarding template.
There are several automations built directly into the template, including:
- Generating a unique SuperForm link when a new item is created and placing that link into the data update link column
- Moving customers between onboarding stages
- Notifying internal teams
- Triggering emails to the customer
- Flagging records that require review
Example Use Cases
Although the video uses a running club as an example of customer onboarding, the workflow can be adapted to many business processes, such as:
Client onboarding
Agencies, consultants, and service providers can use the workflow to collect client details, project requirements, legal and budgetary confirmations, access information, and kickoff materials.
Vendor onboarding
Operations and procurement teams can use the same customer onboarding template to collect vendor details, tax information, compliance documents, contact information, payment details, and signed agreements.
Membership registration
Clubs, communities, fitness businesses, and training programs can use this template to collect member details, medical information, waivers, and consent forms.
Compliance intake
Teams that need complete, signed, and properly stored documentation can use this workflow to reduce missing records and keep evidence attached to the relevant items.
What’s Included in the Template
The ready-to-use customer onboarding template is available from the monday template center and includes the complete SuperForm setup and the preconfigured automations discussed this use case.
The template includes:
- A pre-built customer onboarding board
- Status-based onboarding groups
- SuperForm setup in update item mode
- Automations to create customer-specific update links
- Required onboarding fields
- Waiver/signature flow
- PDF export setup
- Preconfigured automations
- Email verification flow with SuperMail
- Data update process
- Customer status movement
Teams can start with the template and adjust the fields, form pages, waiver text, required questions, statuses, and automations to match their own process.
This customer onboarding template was built to be used as an out-of-the-box solution or customized easily.
For example, a vendor onboarding workflow might replace the medical questionnaire with banking details, insurance documentation, tax forms, and compliance declarations.
A SaaS client onboarding workflow might collect account setup details, admin contacts, product requirements, training preferences, and kickoff availability.
A membership onboarding workflow might collect participant details, emergency contacts, policy acknowledgements, and signed waivers.
The structure stays the same: one customer or vendor record, one guided form experience, one organized monday.com workflow.
